The meeting provides a confidential, non-punitive, and multidisciplinary platform for critical analysis of patient management processes, diagnostic decisions, therapeutic interventions, and system-level factors contributing to clinical outcomes. Through evidence-based discussion and reflective practice, the forum aims to identify preventable factors, strengthen clinical decision-making, enhance patient safety, and promote continuous quality improvement in healthcare delivery.

Objectives

By the end of the session, participants will be able to: 1. Review and analyze selected morbidity and mortality cases using structured clinical audit principles. 2.
